According to the Los Angeles County Department of Medical Examiner-Coroner, Adam died on April 11 from accidental multiple drug intoxication, PEOPLE confirms.
In a July 7 autopsy report obtained by PEOPLE on Wednesday, a medical examiner said that fentanyl, a powerful opioid; flu alprazolam, a known tranquilizer; mitragynine, a psychoactive ingredient found in the kratom plant; ethanol and cocaine were found in Adam’s body.
“Fatal levels of fentanyl” were present in his blood, the medical examiner noted in the document.
